Assist the patient with developing activities and facilitate maximum self-care and independence within the patient’s physical limitations. Assist patient/caregiver with practical skills of everyday living and assess the need for adaptive equipment
Certified nursing assistant (CNA) Assists the patient with meal preparation, personal care, hygiene, light housekeeping, ambulation, and exercises under professional supervision. They can provide various personal care services to help with recuperative and rehabilitative processes
Assist with community resources and long-range planning. Provides information and support regarding financial, emotional, and social aspects of the patient’s illness
Assist patient with speech / language disorder, which may have been impaired following illness or accident. Assist patient with communication techniques as well as problems with cognition and swallowing
Assess nutrition status and assist with individualized meal planning. Teach and instruct patient, family, and caregivers techniques on how to implement the dietary plan of care.
